Referral Pathways That Respect Choice and Control

Ada Support welcomes referrals from participants, families and carers, Support Coordinators, Local Area Coordinators (LACs), NDIA planners, allied health professionals, hospitals, and community organisations.

Our referral approach follows NDIS best practices: it is participant-centred, supports informed choice, protects privacy, and ensures information is collected only where necessary to assess suitability and safety.

If you are unsure what information is required, you can submit a basic referral first and we will contact the participant (or their authorised representative) to confirm details and next steps.

NDIS Referral Best Practices We Follow

Choice and Control

Referrals should be made with the participant's knowledge and agreement, and should reflect the participant's goals and preferences.

Minimum Necessary Information

Only share information required to understand support needs and assess suitability. Avoid including sensitive health details unless it is essential for safe service delivery.

Privacy and Confidentiality

We handle personal information in line with privacy obligations. Where additional documentation is needed, we can request it securely after initial contact.

Risk-Aware, Safety-Focused Intake

We ask about risks relevant to service delivery (e.g., mobility, behavioural support needs, clinical considerations) to support safe planning and appropriate staffing.

Collaborative Communication

With consent, we collaborate with support coordinators, families, carers, and relevant professionals to support continuity and coordinated care.

You confirm that the information you provide is accurate to the best of your knowledge and is limited to what is necessary to assess support needs and deliver services safely.

What Happens Next

Referral Review

We review the referral to understand goals, requested supports, location, and any safety considerations required for appropriate service delivery.

Consent Check and Contact

We confirm that the participant (or authorised representative) has agreed to the referral and that we can contact them to discuss supports.

Service Match and Availability

We assess suitability and availability, including staffing requirements, location coverage, and any accommodation needs (SIL/Respite/SDA).

Intake and Commencement

Where appropriate, we complete an intake process, agree on supports and schedules, and plan a safe start date.
